How to View Responses

Responses can be reviewed in Submitted Forms in two ways: in bulk (all responses together) or per submission (individual entries). By default, all user roles can view responses — but this can be changed in the Privileges section to limit who has access.

Who Can See Submissions?

System Admins

  • Can see and access all forms and submissions (both global and local).

Managers and Users

  • Can see and access local forms and submissions for their facility/clinic.
  • Do not have access to global forms unless created at their location.

Privileges Settings

  • By default, responses are visible to all user roles.

Admins can adjust this in the Privileges section to restrict or expand who can view submissions.

Where to Find Responses

  • From the main menu, go to Reports > Submitted Forms
  • A list of published forms available to you will appear, based on your visibility and privilege settings
  • The Responses column shows the total number of submissions for each form

Viewing Responses Per Submission

  • From the ⠇ menu, select View Submissions 
  • A dynamic submissions table will open:

    • Each row = one submission
    • Each column = a form field

✔ A checkmark indicates a submission includes a digital signature
📎 A paperclip indicates the submission includes an attachment (file upload)

Best Practices

  • Always double-check that the number of forms attached matches the tags you use.
  • Use clear wording in your message so recipients understand which form they are being asked to complete.
  • Keep form names concise so hyperlinks look clean in the final message.
