How to Create a Form
Creating a form or survey is simple and flexible. You can start from scratch and use the drag-and-drop editor to build exactly what you need.
Creating a New Form
- Click on Quick Actions in the top right corner
- Select + Start From Scratch.
- The Form Editor will automatically open

Using the Form Editor
The Form Editor is designed for flexibility and ease of use:
Form Title
- The first step is to give your form a Form Title.
- Once entered, a unique Form Name is automatically generated for tracking and organization purposes.
Drag-and-Drop Builder
- Add form elements (fields, questions, sections) by dragging them into place.
- Customize each field with labels, instructions, or required settings.
- Rearrange items at any time by dragging them into a new order.
Saving Your Work
- You can save the form as a Draft and return to it later.
- When you’re ready, click Publish to make the form live and start collecting responses.
Preview Your Template
To see what the live version of your form or survey will look like:
- Click on the Preview tab at the top of the Editor
- A new tab will open, allowing you to preview your work
- Note, you must Save a draft before you can preview
Best Practices
- Always add a clear title so the Form Name is meaningful and easy to identify later.
- Plan your structure before adding fields to save time.
- Test in Draft mode to ensure everything looks right before publishing.
